What happens when you apply:

(1) By submitting your application, you agree to our Terms & Conditions above.

(2) Once your application is approved, you will receive an email detailing 3 simple steps for finalizing your participation with us. 

(3) Within five business days, you will also receive by mail a receipt for your startup fee, your own Eclectic Health discount card, some administrative guidelines, and a printed copy of our Terms & Conditions.
 
(4) You'll also receive by email 10 suggestions for how to make the most of your Eclectic Health membership.
